Indications/Uses
Vasospastic angina pectoris (Prinzmetal's, variant or at-rest angina), chronic stable angina pectoris (effort angina), essential HTN, hypertensive emergency, Raynaud's syndrome.
Dosage/Direction for Use
Individualized dosing. Starting dose: 10 mg tds. Usual dose: 10-20 mg tds. Titration interval: A least 3 days between dose increases. Patients w/ coronary artery spasm 20-30 mg tds-qds. Max daily dose: 120 mg.
Administration
May be taken with or without food: Avoid grapefruit juice.
Contraindications
Unstable angina pectoris, acute MI (w/in the 1st 4 wk), secondary prevention of MI. Pregnancy & women of child-bearing potential.
Special Precautions
Increased angina during β-blocker w/drawal. Taper β-blocker dose before treatment initiation. Possible severe hypotension & cardiac output lowering in patients w/ severe heart failure. Patients w/ heart failure; severe aortic stenosis. Increased CV complications (eg, MI) & mortality; only use for patients w/ essential HTN & chronic angina pectoris if other drugs are not indicated. Monitor BP & heart rate carefully during therapy. Closely observe patients concomitantly taking BP-lowering medications. Potential mild to moderate peripheral edema typically associated w/ arterial vasodilation; differentiate this peripheral edema from the effects of increasing left ventricular dysfunction in patients w/ CHF-complicated angina. May require adjustment in diabetic patients. Elderly (especially to those w/ history of hypotension or cerebral vascular insufficiency).
Adverse Reactions
Headache; dizziness, lightheadedness, giddiness, nausea, vomiting, GI distress, flushing, heat sensation; peripheral edema, hypotension.
Drug Interactions
Antihypertensive effects of β-blockers may be augmented. May potentiate the effects of hypotensive agents. Almost double peak plasma levels & possible exacerbation of effects (eg, hypotension) w/ cimetidine.
